For a large regional health system, created the vision and roadmap and secured funding for the 5-year population health management strategy, spanning new enablement capabilities, new physical and virtual care models, and new medical management actions.
For a state based plan – led the analysis, design and negotiation of 2 value-based partnerships between the health plan and two providers; an integrated health system and a large multispecialty physician group.
For a national provider, evaluated strategic and financial feasibility of launching an insurance product and identified target markets and target payers for value based care partnerships.
For a national retail pharmacy chain, supported the pilot launch and led the subsequent national expansion of a new business model to partner with physician groups and health systems to engage and manage polychronic patients and capture value-based revenue.
